  I flew to the last OPTN/UNOS board meeting in the company of Dr. Phil Camp, chair of the Operations and Safety Committee.  Despite his frustrations with the pace of progress, I was very pleased with his energy, attitude, and goals for improvement of the organization as well as encouraging improvement at the OPO and transplant center level.  This newsletter is a sign that he's making progress.  Go, Phil!

Top 5 policy issues identified during UNOS Site Surveys:
1. Documenting Patient Notification
2. Documenting ABO verification before transplant
3. ABO typing X 2 prior to listing candidates
4. Data entry errors of candidate status or errors in documenting status
5. Not removing recipients from the waiting list within 24 hours of transplant
